Title: RN/LPN – Mobile MAT Unit
Hours Available: 4 x 10 Hour Shifts
Location: Lakewood, CO
This position is part of a mobile medication assisted therapy (MAT) unit funded by the State of Colorado to provide mental health and co-occurring substance use services to consumers in the rural areas of Clear Creek, Gilpin and Jefferson counties. Position will provide psychiatric and brief medical nursing services, monitor in-home induction and other medication services and will collaborate with the MAT team (clinician, peer specialist and Psychiatrist) to coordinate care. This position will serve adults and adolescents aged 15 and older within the identified communities; additional subpopulations to be served include homeless/transient individuals. This position will screen and assess individuals for potential physical conditions impacting mental health and substance use of consumers. The ideal candidate will have knowledge regarding harm reduction including supportive services and strategies to enhance the knowledge, skills, resources, and supports for individuals, families and communities to be safer and healthier.
