Integrated Behavioral Health Clinician (LICSW, LCSW or LMHC) Opportunity in Worcester, MA with PCMH Prime Level III Accredited Team

We are hiring an experienced (2years+ grad) Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) to complete assessments, brief interventions, care coordination, and individual, family, and group behavioral health interventions with children, adolescents, and adults for our client in Worcester, MA.

You will join a PCMH Prime Level III Accredited team that has received the prestigious HRSA Gold and Silver Award for quality client service. POSITION SUMMARY:

The Clinician will perform a variety of duties as a member of the Patient-Centered Medical Home, Primary Care Team. Work will include consultations, brief assessments, brief interventions, care coordination, and individual, family, and group behavioral health interventions with children, adolescents, and adults. The Clinician will work directly with medical providers and the care team to collaboratively address patients’ care and to support optimal medical and behavioral health.

REQUIREMENTS:

  • Candidate must be eligible for and actively working towards obtaining an LCSW, LICSW or LMHC, or other similar credentials.
  • Two (2) to four (4) years direct patient care and managed care experience preferred; public sector experience may be applicable. This experience may be in an internship.
