POSITION SUMMARY:
The Clinician will perform a variety of duties as a member of the Patient-Centered Medical Home, Primary Care Team. Work will include consultations, brief assessments, brief interventions, care coordination, and individual, family, and group behavioral health interventions with children, adolescents, and adults. The Clinician will work directly with medical providers and the care team to collaboratively address patients’ care and to support optimal medical and behavioral health.
